Ages 5 to 26 Special Education Survey

Dear Parent,

Special Education is intended to help families of students ages five to twenty-six with disabilities get the services and supports they need. We have been asked by the Michigan Department of Education to help find out families' views about Special Education through this online questionnaire.

Would you help us by filling out this questionnaire?This is your chance to tell us your thoughts and feelings about Special Education and about the services your family has received. Your responses will help guide efforts to more successfully involve families in their children's education across Michigan.

The questionnaire is voluntary and confidential. This means that you do not have to fill out the questionnaire, but we will ensure your privacy if you do. We never report the answers of individual families to anyone. All of the information we collect will be reported as group results. If you prefer, we can help you complete the questionnaire over the phone if you call us at1-888-800-8865.

To complete the following on-line questionnaire, you will need to type in the code number found on the top of the first page of the questionnaire that was mailed to you. [The code numbers will help us ensure that we are not receiving duplicate surveys.] You are asked to only complete ONE questionnaire - either the online version OR the paper copy you received in the mail (not both).
