If you have overdosed from heroin or opioids, or if you have encountered someone while they were experiencing an overdose emergency completing this anonymous questionnaire is very important.
Our program aims to stop overdose deaths in Southeast Michigan communities by preparing people to intervene during an opioid overdose emergency to save a life. The information you provide will help us:
- learn more about overdose emergencies occurring in the region and what support is most needed
- better understand how people respond during an overdose emergency
- improve how we prepare people to respond during an overdose emergency
- show the importance of overdose prevention and response programs
- ultimately, save more lives.
You are not required to provide your name or any other identifying information. All responses are completely anonymous. It will take about 15 minutes to complete the electronic form.
If you have encountered more than one overdose emergency (more than one person, date, or location), please fill out a separate form for each occurrence.
